Eligibility
Middle & High School Eligibility
The Ohio High School Athletic Association governs the eligibility requirements for high school and middle school athletes in the state of Ohio. For information on Ohio’s eligibility standards and specific sport rules, visit the OHSAA website.
College Eligibility
Athletes considering participating in an NCAA member school in college must meet NCAA Eligibility Standards, and must be registered with the NCAA Clearinghouse. Student athletes should also contact their school counselor for support.

Participation Forms
We are very excited that the Reynoldsburg Athletic Department has partnered with FinalForms, an on-line program which will enable parents and students to complete & file MOST athlete forms electronically. FinalForms is immediately available for all Raider athletes. The ONLY form that you are still required to actually print, complete, and file in the Athletic Department, is the actual OHSAA Physical form. The physical form must be completed by the Doctor and by law we are required to have a signed copy on file. Once your student’s physical is received by the athletic department we will input the expiration date into FinalForms and then you will receive automatic notifications when your student’s forms need to be updated.
We are asking that ALL student-athletes (grades 7-12) use FinalForms. Even if your student’s current physical paperwork does not expire until after the new school year begins, please register at https://reynoldsburg-oh.finalforms.com now and electronically complete all the forms!
If you have any problems getting logged on, registered, or completing your forms – contact your coach or the Athletic Department and we can send you a step-by-step guide via e-mail.
While all forms can be completed electronically, pages 4 and 5 of the Physical Form (one single sheet front and back) must be signed by the Doctor and that is the only hard copy of paperwork needed. Participation Fees
Junior High (Grades 7–8): $400 per student-athlete, per sport
High School (Grades 9–12): $500 per student-athlete, per sport
There are no athletic fee “waivers” for Free and Reduced lunch qualifiers

Junior High and High School Payments may be made at each of our high school campuses. Please verify the location by date below.

- Tuesday, June 16 – Thursday, June 18: 11:00am – 2:30pm at the RHS Athletics Office
- Tuesday, June 23 – Thursday, June 25: 11:00am – 2:00pm at the RHS Athletics Office

*When considering a payment by mail, please be aware that mail will not be delivered to the Reynoldsburg High School - Livingston Campus from June 10, 2026 until approximately August 3, 2026.
Payment Options
One Time Full Payment
Fees may be paid in full by check (no postdated checks), money order, cash (exact change) or online with a credit/debit card. Full payments may be made by credit/debit card by accessing your student-athlete’s Final Forms account. Card payments cannot be taken in the Athletics Office.
*All full payment Pay to Participate Fees are due by the first payment date for your sport season.

Two Payment Option
Payment option is for SPORTS ONLY. This does not include band, choir, orchestra, etc.
Junior High School:
Payment 1: $200 due by first due date for your sport
Payment 2: $200 due by second due date for your sport
High School:
Payment 1: $250 due by first due date for your sport
Payment 2: $250 due by second due date for your sport
Note: The two payment option is not available online. Forms of payment are cash (exact change), check (no postdated checks) or money order.
**All partial payments must equal (no more, no less) $250 for HS, $200 for JH. Please note that if any portion of the payment plan is not received in the office by the due date, student-athletes will not be able to participate until payment is received.
Code of Conduct
Directions
The Varsity Football Stadium/Track, Fieldhouse and Gymnasium are at Reynoldsburg High School – Livingston Campus.
Parking is available in the east and west parking lots for all events.
Indoor events should use the east lot, which is closest to the gymnasium.
The Varsity Gymnasium for Basketball, Varsity Softball Field, and tennis courts are at the Reynoldsburg High School – Summit Campus. Parking is available in the north parking lots for all events.
The Varsity Soccer Stadium and Varsity Baseball Field are at Baldwin Road Junior High.

Turn Right on Summit Rd. (East of Taylor Rd.), campus is located on the right
